ABSTRACT

Objective:To explore the clinical effects of intramuscular injection of oryzanol for 5 and 10 days in the treatment of menopausal syndrome.Methods:This study adopts a case-control study method.Twenty patients with menopausal syndrome admitted to Peking Union Medical College Hospital from July 2021 to March 2022 were selected and randomly divided into a control group and an observation group using a random number table method, with 10 patients in each group. The control group was given intramuscular injection of oryzanol once a day for 5 consecutive days on the basis of routine health education; On the basis of routine health education, the observation group was given intramuscular injection of oryzanol once a day for 10 consecutive days. Two groups of patients completed a total of 6 visits before treatment and on days 5, 10, 30, 60, and 90 of treatment. Baseline survey was conducted to record the basic information of patients, including demography data, menstruation, past history, surgical history, drug allergy and the main symptoms of climacteric syndrome, and the baseline hormone level of patients was detected. Compare the menopausal symptoms by modified K-score, Pittsburgh sleep quality index (PSQI) score, menopausal specific quality of life (MENQOL) score, post treatment injection site pain by visual analog score (VAS), and observe adverse reactions during treatment between the two groups of patients at each visit time point.The measurement data conforming to the normal distribution is expressed in xˉ± s. The comparison between the two groups is performed by t test, and the change trend of the two groups at each time point is compared by repeated measurement analysis of variance; The measurement data that do not conform to the normal distribution are represented by M( Q1, Q3), and the Kruskal Wallis test is used for comparison between groups; Counting data is represented by examples(%), Fisher's exact probability method is used for inter group comparison, and Wilcoxon rank sum test is used for rank data comparison. Results:The body mass of the control group was higher than that of the observation group [(62.50±11.88) kg vs (52.90±4.28) kg, t=2.40, P=0.027]. There was no statistical difference between the two groups in other demography data and previous history, sex hormone levels, menopausal symptoms, and menstrual conditions (all P>0.05). Compared with the baseline, the modified K-score, PSQI score, and MENQOL vascular dilation and contraction, psychological state, physiological state, and sexual life scores of the two groups of patients on the 5th, 10th, 30th, 60th, and 90th day of treatment were significantly reduced, and showed a gradually decreasing trend with the extension of observation time (the values in Group F were 36.37, 12.40, 18.69, 24.73, 40.77, and 18.39, respectively, P<0.001), but there was no statistically significant difference between the groups (all P>0.05). After treatment, the VAS score of the control group was 1 point in 2 cases, 2 points in 4 cases, and 3 points in 4 cases; The VAS score of the observation group was 1 in 4 cases, 2 in 3 cases, and 3, 5, and 6 in 1 case each. There was no statistically significant difference in VAS scores between the two groups ( Z=0.43, P=0.664), and no adverse events or serious adverse events were observed during the treatment period. Conclusions:Intramuscular injection of oryzanol injection for 5 and 10 days can improve menopausal symptoms, improve sleep quality and overall quality of life in patients with menopausal syndrome, and the overall effect is similar.

ABSTRACT

Menopausal syndromes are mostly based on kidney deficiency, which could be expalined that kidney governing essence storage and controlling innateness, so when the kidney water was deficient and the liver fail to nourish, then led to liver depression and transform into fire; deficiency of the kidney, loss of warmth of the spleen, and inability to transport and transform the water and dampness will easily lead to phlegm and fire; the decline of the kidney yin and loss of water and fire will easily cause the exuberance of heart fire. Therefore, clinical symptoms of hot flashes, insomnia, and palpitations are common due to phlegm, depressions, and fire. Based on this, at the beginning of the treatment, we should treat the symptoms firstly by resolving phlegm, relieving depression and clearing fire, and commonly use Huanglian Wendan Decoction (黄连温胆汤), Yigan Powder (抑肝散), Chaihu plus Longgu Muli Decoction (柴胡加龙骨牡蛎汤), and Qingxin Zishen Decoction (清心滋肾饮), etc. After improving the symptoms of hot flashes and sweating, irritability, dreaming and frightening, then we should give the prescriptions to tonify kidney yang and nourish kidney yin, in order to eliminate the pathogens and reinforce healthy qi, and to treat both the manifestations and the root cause, so that the symptoms of the patients can be better alleviated.

ABSTRACT

We report a case of a 49-year-old woman who presented with 6-month period menopausal syndrome including excessive sweating and hot flushes in her face, chest and back. She was also frustrated with her sons, but she reflected on her own anger and wondered why she got so irritated with them. We prescribed yokukansan to her and her all symptoms improved one month later. We need to differentiate between yokukansan and kamishoyosan in the treatment of menopausal syndrome and irritation, and it may be useful to evaluate the detail of irritation in selecting formula.

ABSTRACT

In order to explore the pathological mechanism of perimenopausal syndrome and seek prevention and treatment measures, it is necessary to establish animal models that similar to human perimenopausal syndrome, so as to provide reference for drug research, new drug development and clinical application. In this paper, the keywords of "perimenopausal period" "perimenopausal syndrome" "menopause" "menopausal syndrome""menopausal period" "menopausal syndrome" and "animal" were searched in China National Knowledge Infrastructure (CNKI), Chongqing Weipu, China Biomedical Literature Database (CBM) and Pubmed. In addition, the selection of domestic peripheral menopausal syndrome model animals in recent years and the advantages and disadvantages of corresponding models were summarized. A total of 673 studies were identified, of which 61 were included in the analysis. The most common animal model of perimenopausal syndrome is castration model, while the immunodeficiency model is less used. With the aging of the population and the rapid increase of psychosocial stress, the incidence of perimenopausal syndrome is high. Therefore, it is particularly important to explore the mechanism of perimenopausal syndrome. According to the experimental purpose, experimental period, experimental technology and other factors, the selection of appropriate model animals and modeling methods is the key of the success of the experiment of perimenopausal syndrome.

ABSTRACT

The coordination and unification of Yin and Yang are the basis of normal human life activities. Along with the age growth and aging of the body, women will suffer from menopausal syndrome during menopause. In addition to the significant changes in the genital system, there are also pathological manifestations in estrogen target points including bone, nerve and cardiovascular systems, due to the imbalance of Yin and Yang. Besides the insufficiency of estrogen, the main cause of menopausal syndrome is the changes in the response of target organs to estrogen. In other words, the biological effects mediated by estrogen receptor(ER) alpha and beta subtypes in target cells are often different or even opposite; the changes of expression level and ratio of ERα and ERβ are also important causes for the abnormal estrogenic effects in target organs and the imbalance of Yin and Yang of the body. Therefore, on one hand, the therapeutic mechanism of drugs is ER-mediated estrogenic effect. On the other hand, the drugs have a regulatory effect on ER subtype expression in target cells and Yin-Yang state in target organs and even organisms, so as to cause further changes in the response of target cells to estrogen or estrogenic components, and exert its therapeutic effects. This paper reviews the pharmacological mechanism of gynecological traditional Chinese medicine in harmonizing Yin and Yang in estrogen-positive target cells and the clinical efficacy in the following aspects, including estrogen and its mechanism, the estrogenic effect of ER in traditional Chinese medicine and the mechanism of ER subtype in balancing Yin and Yang and mediating and regulating the main target tissues in menopausal syndrome treatment.

ABSTRACT

Objective:To observe the clinical efficacy of addition and subtraction syndrome differentiation method of Huanglian Ejiaotang and Baihe Dihuangtang in the treatment of menopausal syndrome (MPS) and symptoms of deficiency of yin and hyperactivity of fire, and investigate its regulation effects on neuroendocrine network. Method:One hundred and forty-eight patients were divided into control group (74 cases) and observation group (74 cases) according to the random number table method. The patients in control group received estradiol/norethisterone acetate tablets, 3 mg/time, 1 time/day, while the patients in observation group received addition and subtraction of Huanglian Ejiaotang and Baihe Dihuangtang based on the same western medicine treatment as control group, orally 1 dose/day, with a treatment course of 3 months in both groups. Kupperman index (KI) score was evaluated before treatment and at 1 month, 2 months and 3 months after treatment. The Menopausal Quality of Life (MENQOL), self-rating depression scale (SDS), Self-rating Anxiety Scale (SAS), Pittsburgh Sleep Quality Index (PSQI) and score of deficiency of yin and hyperactivity of fire were evaluated before and after treatment. The levels of serum estradiol (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH), norepinephrine (NE), 5-hydroxytryptamine (5-HT), nitric oxide (NO), endothelin-1 (ET-1) and calcitonin gene-related peptide (CGRP) were detected before and after treatment. Result:The clinical efficacy in observation group was better than that in control group (Z=2.115, P<0.05), and the KI scores in observation group at 1 month, 2 months and 3 months after treatment were lower than those in control group (P<0.01), and the decrease of KI score in observation group after treatment was larger than that in control group (P<0.01). The scores of various dimensions of MENQOL scale in observation group were lower than those in control group (P<0.01). The scores of deficiency of yin and hyperactivity of fire, SAS, SDS and PSQI in observation group were all lower than those in control group (P<0.01), and the level of E2 in observation group after treatment was higher than that in control group (P<0.01) ,while the levels of FSH and LH were lower than those in control group (P<0.01).The levels of 5-HT and NE in observation group were higher than those in control group (P<0.01).The NO level in observation group was higher than that in control group (P<0.01), while the levels of CGRP and ET-1 were lower than those in control group (P<0.01). During the study period, there were 5 drop-out cases, 2 excluded cases, and 67 completed cases in the control group. There were 7 drop-out cases, 1 excluded case, 66 completed cases in the observation group. Conclusion:On the basis of hormone replacement therapy, addition and subtraction of Huanglian Ejiaotang and Baihe Dihuangtang for patients with MPS (with symptom of deficiency of yin and hyperactivity of fire) can improve clinical symptoms, relieve depression and anxiety, improve sleep quality and enhance quality of life, and its clinical efficacy is superior to hormone replacement therapy alone. It can also regulate endocrine hormones, vasomotor factors and monoamine neurotransmitters, thereby improving clinical symptoms.

ABSTRACT

A prospective study of 1484 women aged 35-70 years was conducted, which were divided into 3 groups -late reproductive, perimenopausal and postmenopausal. Menopausal syndrome (MS) was detected in 1369 (93.3%) women. Every third woman in peri-and postmenopause had a moderately severe MS. The metabolic imbalance worsened more as the severity of the MS increased, rather than with increasing age. Predictors of pathological menopause are identified -these are the main components of the metabolic syndrome (diabetes mellitus, hypertension, obesity); uterine fibroids, as well as urbanization and higher education. The protective factors for the development of the MS was the absence of the fact of marriage (single), medical education and waist circumference of 80 cm or less.

ABSTRACT

OBJECTIVES: To investigate the relationship between insomnia (INS) combined with chronic musculoskeletal pain (MSP) in postmenopausal women and its characteristics regarding MSP, menopausal and mood symptoms, sleep and quality of life (QOL). METHODS: A cross-sectional control study in 4 groups of postmenopausal women: control (n = 15), MSP (n = 15), INS (n =15) and INS + MSP (n = 17). Sixty-two participants completed questionnaires and had blood collected, and 43 underwent polysomnography. RESULTS: INS was associated with increased anxiety (P = 0.04) and sleep fragmentation (P = 0.02); worse MSP severity (P = 0.00), MSP interference with daily function (P = 0.00), higher pain intensity at midday (P = 0.02) and menopausal symptoms (P = 0.00); and reduced QOL (P = 0.00). MSP was associated with increased anxiety (P = 0.02) and menopausal symptoms (P = 0.00), and reduced QOL (P = 0.05). In the whole sample, depression symptoms were higher but no statistical differences were found between groups (P = 0.47). Worse QOL was associated with both higher depressive symptoms (P = 0.01) and worse pain interference (P = 0.02) CONCLUSIONS: INS + MSP was related to higher menopausal and anxiety symptoms, more sleep fragmentation and complaints of MSP severity and interference, more pain sites and worse QOL. The presence of INS was associated to more MSP. Sleep management is essential in women who have developed chronic MSP.

ABSTRACT

OBJECTIVE: To observe the clinical efficacy of joint application of electroacupuncture (EA), acupointinjection and fire-needle in the treatment of menopausal syndrome (MS) women. METHODS: One hundred MS patients were randomized into control group and combined treatment group (n=50 patients in each). The combined treatment group was treated by joint application of EA, acupoint-injection and fire-needle stimulation, and the control group treated by EA stimulation alone. EA was applied to Shenshu (BL 23), Xinshu (BL15), Taixi (KI 3), Sanyinjiao (SP 6), Laogong (PC 8), Pishu (BL 20), etc., for 30 min, once daily for 10 days. Point-injection of red ginseng-ophiopogon root injection (0.5 mL) or red-hot needle stimulation was applied to bilateral BL15, BL 20, SP 6 and BL 23 once daily for 10 days and once every other day for 5 times, respectively. The modified Kupperman index scale was used to evaluate symptoms (hot flashes and night sweats: 4 points; paresthesia, insomnia, dyspareunia, irritability and problems of urinary system: 2 point each; depression, dizziness, fatigue, joint pain or muscular pain, headache, palpitation, skin formication: 1 point each) of the perimenopausal syndrome, and 3 months' follow-up survey was conducted after the treatment. Each symptom score was calculated as the product of the symptom severity score (0, no; 1, mild; 2, moderate; 3, severe) multiplied by the points of symptom index. The total Kupperman score was the sum of the scores of all the items. The therapeutic effect was also evaluated according to the "Guiding Principles for Clinical Trials of New Drugs of Chinese Materia Medica" (2002). RESULTS: The Kupperman scores were significantly decreased following the 10 days' treatment and 3 months thereafter in both control and combined treatment groups in comparison with their individual pretreatment (P<0.05), and were obviously lower in the combined treatment group than in the control group after treatment (P<0.05). Of the both 50 patients in the control and combined treatment groups, 22(44%) and 35 (70%) experienced a remarkably improvement, 18(36%) and 12(24%) were effective, and 10(20%) and 3 (6%) ineffective, with the total effective rates being 80% and 94%, respectively. The therapeutic effect of the combined treatment group was notably superior to that of the control group (P<0.05). CONCLUSION: The joint application of electroacupuncture (EA), acupoint-injection and fire needle is significantly superior to that of simple EA in improving the symptoms of patients with menopausal syndrome.

Objective To investigate the clinical effect of combined therapy of left and right amlodipine in the treatment of hypertension complicated with perimenopausal syndrome. Methods 50 patients with hypertension associated with peri menopausal syndrome in our hospital from January 2015 to January 2016 were selected,and according to the different treatment and nursing intervention divided into observation group and control group,25 cases in each groups. The control group with routine nursing intervention and benazepril treatment, observation group with amlodipine and comprehensive psychological nursing intervention treatment before and after treatment in two; blood pressure, luteinizing hormone (LH), serum follicle stimulating hormone (FSH), estrogen (E2) level and the changes of anxiety and depression (SAS) (SDS) were recorded in detail, and the relevant data for comparative analysis. Results Amlodipine+comprehensive psychological nursing intervention (observation group) of peri menopausal comprehensive clinical effect is better than that of benazepril+routine nursing intervention patients with hypertension treatment (control group) clinical efficacy, clinical symptoms improved better than the control group, the difference was statistically significant (P<0.05). Conclusion Hypertension patients with perimenopausal syndrome patients choose amlodipine + comprehensive psychological nursing intervention treatment effect significantly, can effectively improve the clinical symptoms and psychological state, and is worthy of clinical application.

ABSTRACT

Objective To study the regulation of serum FSH and E2 in the treatment of menopausal syndrome with 17 beta estradiol combined with DRSP. Methods A total of 90 female patients with menopausal syndrome treated in our hospital from November 2013 to November 2016 were selected. The patients were randomly divided into three groups: 17β group (30 patients, 17β-estradiol alone), DRSP group (30 patients treated with DRSP alone) and 30 patients (17β-estradiol combined with DRSP). Kupperman's menopausal syndrome was used to evaluate the physical symptoms of the patients and to compare the patients after the treatment. The patients' treatment efficiency was observed. The items examined before treatment included liver and kidney function, hematuria routine , Fasting blood glucose, FSH, E2, LDL, blood TC, electrocardiogram, intracavitary Doppler ultrasound, breast ultrasound, HDL, Kupperman evaluation table in accordance with the evaluation before the end of each treatment measure again, And performs the above-mentioned inspection. Results In the combined group, there was no significant difference in the scores of other symptoms except the differences between the ants on the skin and the treatment before treatment. There were significant differences in the scores of pain, headache, body sensation, agitation, palpitation, Pain, fatigue, insomnia and hot flashes were significantly different (P<0.01). In the DRSP group, pain, depression and insomnia were improved after treatment (P<0.05), other symptoms were (P<0.05), other symptoms were not improved;the total effective rate of combined group was 90.0%, DRSP (P<0.05), and the total effective rate of the group was significantly higher than that of the control group (P<0.05) .There was no significant difference The total effective rate was 73.33% in the group and 70.0% in the 17β group. There was significant difference between the combined group and the other groups (P<0.05). The difference of TC, E2 and FSH in the combined group before and after treatment (P<0.05). There was no difference in FSH between the two groups before and after treatment. There was no difference between the two groups before and after treatment. Conclusion 17β-estradiol combined with DRSP in the treatment of menopausal syndrome patients achieved good clinical effect, the serum levels of FSH and E2 in the body is significant.

ABSTRACT

Objective To study distribution rules of TCM syndrome characteristics of menopausal syndrome; To provide references for clinical treatment and further study.Methods The articles related to menopausal syndrome in CNKI, VIP, Wanfang Data, CBM and TCMDS during Jun. 1983 to Jun. 2013 were retrieved and analyzed. The original data were input into computer and established the menopausal syndrome literature database. Methods of cluster analysis, factor analysis, and frequency analysis were combined to conduct statistical analysis.Results A total of 270 articles and 730 patients were included in the analysis, including 42 syndromes and 73 symptoms. The four diagnostic information of top 9 syndromes was analyzed by clustering and 5 kinds of basic syndrome types were obtained, including liver and kidney yin deficiency, liver depression, heart-kidney disharmony, heart and spleen deficiency, and spleen and kidney yang deficiency; the core symptoms of each syndrome were extracted by factor analysis and frequency dominance method, and the main points of syndrome differentiations were initially established. Conclusion Three methods combined with relevant expert advice were used to initially establish the syndrome diagnosis of menopause syndrome, which can lay a solid foundation for further study.

The number of women who reduced dosage of tibolone in the treatment group was more than in the control group (P<0.05). Conclusion Kuntai capsules combined with tibolone is better in treating menopausal syndrome than tibolone treatment.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the efficacy differences between acupoint catgut embedding combined with auricular point pressure with beans and nilestriol on menopausal syndrome of liver-kidney deficiency type, and to explore their effects on estradiol (E).</p><p><b>METHODS</b>Sixty patients with menopausal syndrome of liver-kidney deficiency type were randomly divided into an acupoint stimulation group and a medication group, 30 cases in each group. The patients in the acupoint stimulation group were treated by acupoint catgut embedding at Taixi (KI 3), Sanyinjiao (SP 6), Shenshu (BL 23), Ganshu (BL 18) and Taichong (LR 3), combined with auricular point pressure at Gan (CO), Shen (CO), Neifenmi (CO), Shenmen (TF), Pizhixia (AT); the treatment was given once a week for consecutive four weeks. The patients in the medication group were treated with oral administration of nilestriol, 1 mg, once a day, combined with oral administration of oryzanol, 20 mg, three times per day for consecutive four weeks. The clinical symptom score was compared between the two groups before and after treatment as well as in follow-up visit. The level of Ewas obserced before and after treatment, and the clinical effect was compared.</p><p><b>RESULTS</b>(1) Compared before treatment, the clinical symptom score in the two groups was significantly reduced after treatment and in follow-up visit (all<0.05); In follow-up visit, the clinical symptom score in the acupoint stimulation group was significantly lower than that in the medication group (<0.05). The different value before treatment and at follow-up in the acupoint stimulation group was better than that in the medication group (<0.05). (2) Compared before treatment, the level of Ein the two groups were increased after treatment (both<0.05); compared before and after treatment, the difference in the treatment group was significantly higher than that in the medication group (<0.05). (3) After treatment, the total effective rate was 93.33% (28/30) in the acupoint stimulation group, which was similar to 90.00% (27/30) in the medication group (P>0.05).</p><p><b>CONCLUSIONS</b>Compared with nilestriol, acupoint catgut embedding combined with auricular point pressure with beans could better improve clinical symptoms for patients with menopausal syndrome of liver-kidney deficiency type, and increased the level of E.</p>

ABSTRACT

Objective To establish the optimum syndrome classification method by using the technology of modern TCM diagnosis and artificial intelligence analysis method for menopausal syndrome differentiation of TCM. Methods Diagnostic information of menopausal syndrome patients was collected and syndromes were classified according to TCM syndrome differentiation standard. Three kinds of common data mining classification algorithm, Bayesian network, K-nearest neighbors and support vector machine, were used for analysis on information data of the four methods of diagnosis of menopausal syndrome.Results The time, classification accuracy, coverage rate and margin curve of establishing TCM syndrome model by the three kinds of algorithm methods under the circumstances of same training and data. The influence of the number of training samples of 3 kinds of algorithm methods was analyzed, and the model established by the three kinds of algorithms was evaluated.Conclusion Bayesian network algorithm is better than the other two methods in the menopausal syndrome classification effect.

ABSTRACT

Objective]The paper summarizes the clinical experience of Professor Lu Su for treating menopausal syndrome.[Methods] By learning from Professor Lu Su, sum up the knowledge of etiology and pathogenesis, treatment characteristics and analyze cases. [Results]Professor Lu Su thought that the main etiological and pathogenesis of menopausal syndrome was that deficiency of the kidney-yin and hyperactivity of heart and liver fire. The kidney water cannot aid heart, the occurrence of heart(including liver) fire too strong, the heart is not calm, leading to hot flashes sweating, upset insomnia and other symptoms of menopausal syndrome. Therefore, she made a total therapeutic principle of invigoration of the kidney and nourishment of Yin, purging heart fire and soothing the nerves. Besides, the article introduces prescribing characteristics of other lesions relating to menopausal syndrome, such as blood stasis, phlegm, yang deficiency and disharmony between spleen and stomach. In treatment, Lu Su is good at syndrome differentiation and treatment, prescribing refinedly and simply, combining medication with psychotherapy. [Conclusion]Therapies of Professor Lu Su on treating menopausal syndrome have significant effect.

Methods Our experiment adopted the HPLC-MS/MS technology of metabolomics researching methods, aiming at the comprehensive change characteristics in metabolism group level of menopausal rat and searching for age-related biomark?ers. The urine of 6 and 18 months rat was taken for HPLC-MS/MS metabolomics analysis. Besides,XCMS online analysis was used for positive and negative ion detection spectrum and the principal component analysis was used to identify biomarkers. Finally ,MetPA was used to implement a Google-map style interactive visualization system,which has analyzed the metabolic pathway. Pathway views were generated dynamically based on interactions with the visualized system. Results The TCA cycle,arginine and proline metabo?lism and aldehyde acid metabolism are related to aging rat metabolite pathways. Conclusion Age-related metabolic pathways are mainly related to the respiratory chain reaction and the urea cycle,which are influenced by liver mitochondrial function.

ABSTRACT

Objective To explore the clinical efficacy and safety of continuous sequential hormone replacement therapy of femoston for menopausal syndrome .Methods 121 cases of menopausal syndrome at expert outpatient service in our hospital from January 2014 to October 2015 received hormone replacement therapy of femoston for 24 weeks .Before and after treat-ment ,Kupperman score ,blood follicle stimulating hormone (FSH) ,estradiol (E2 ) ,luteinizing hormone (LH) and endometri-al thickness were determined .Results With the prolonging of treatment time ,patient's menopausal symptoms gradually re-lieved .Kupperman scores were declining .After 6 courses of treatment ,patient's menopausal symptoms completely eased . Kupperman scores decreased by 95 .0% (P<0 .01) ,serum FSH and LH decreased significantly (P<0.05 and P<0 .01) and serum E2 increased notably (P<0 .01) without endometrial thickness changes (P>0.05) .Conclusion Continuous sequential therapy of femoston can effectively relieve menopausal syndrome and control the menstrual cycle and the endocrine level without stimulating the endometrium excessively .
